Consultant Evaluation

Overview

For sponsors choosing to use consultants, we ask that you complete a consultant evaluation form while the work is being performed or at the completion of the work being performed.  All consultants must be prequalified in the area in which they are performing work. In order for us to assess performance, the sponsor should complete the evaluation form, and submit to the TE Coordinator.  Please send completed evaluation form to:
